I'm assuming you're talking about the following:
If you're playing as a male, I've read that there is never a face mask. Apparently it's a female-only feature. Whether or not this is a bug/glitch has not yet been answered by Bethesda.
To summarize, in order to have the mask, you must satisfy the following:
